📋 Description

• Provide support to the manager by precepting, coaching, and mentoring team members • Guide, support, and develop team processes and maintain a positive work environment • Provide field work support and monitor field work visits completed by team members • Escalate issues to the manager and support team management as requested • Provide coverage for Care Coordinators 1 and 2 and document in the clinical system • Mentor and support the team and training initiatives • Provide daily reports on outstanding cases and review daily compliance dashboards • Support audit initiatives and internal and external auditing processes • Complete field work support and review reports • Support new-hire onboarding, including precepting and case audits • Coordinate services and monitor and evaluate case management plans against members’ personal goals • Monitor patient care through assessments, data, conversations with members, and active care planning • Guide members and families toward appropriate care resources • Facilitate interactions with payer sources, providers, interdisciplinary teams, and others involved in members’ care • Visit Medicaid members in their homes, long-term care facilities, and other care settings • Support either MLTSS or LTSS based on business needs

🎯 Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field, OR unrestricted Illinois LPN with 1 year of experience conducting comprehensive assessments and providing formal services for the elderly, OR Licensed Professional Counselor (LPC), OR Licensed Social Worker (LSW) • Must reside in the State of Illinois • 2+ years of experience in health care and/or case management • 2+ years of experience with Medicare and Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations • Ability to use electronic information applications and software programs, including electronic medical records • Must complete tuberculosis screening if selected • Valid state driver’s license • Proof of personal vehicle liability insurance with limits of at least 100,000/300,000/100,000 • Patient-facing role subject to Humana’s TB screening program
