📋 Description

• Manage members' health issues using telephonic strategies, approaches and techniques • Identify and resolve barriers to effective care • Conduct assessments and monitor patient care and progress toward desired outcomes • Use data, conversations with members, and active care planning to support care • Guide members and their families to essential health and community resources • Apply professional concepts, standards, regulations, strategies and operating standards • Make decisions regarding work approach and priorities while following direction and established procedures • Report to the Manager, Care Management • Join a dedicated team supporting compassionate, patient-centered service

🎯 Requirements

• Bachelor's degree in a health-related field, active Illinois LPN licensure, or the ability to obtain an IL LPN license • 2 or more years of case management, medical case management, care coordination, or healthcare support experience • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook • Previous experience with electronic case note documentation • Experience documenting in multiple computer applications/systems • Experience with health promotion, coaching and wellness • Knowledge of community health and social service agencies and additional community resources • Ability to work Monday–Friday within operating hours of 8:00 AM–7:00 PM CST, with core hours primarily 8:00 AM–5:00 PM CST • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
