📋 Description

• Coordinate care for patients with varied healthcare needs • Assess patients’ healthcare needs and complete telephonic assessments • Review patient information and clinical documentation • Identify potential barriers to care and develop appropriate individualized care plans • Provide education to patients and caregivers and answer questions about plans of care • Collaborate with physicians, home health clinicians, hospitals, and other healthcare professionals • Monitor patient progress and identify changes in condition • Coordinate services and help patients access available healthcare resources • Use clinical judgment to recognize potential concerns and escalate them • Document patient interactions accurately and timely • Maintain confidentiality and follow applicable policies and regulatory requirements

🎯 Requirements

• Active and unrestricted Registered Nurse license • 3–5 years of clinical acute care experience • At least 2 years of home health experience • Experience reviewing clinical information and using nursing judgment to identify patient needs • Experience working with electronic medical records and other computer-based healthcare systems • Previous experience in care management, case management, utilization management, or telephonic nursing preferred • Experience coordinating care between hospitals, physicians, home health agencies, and other healthcare providers preferred • Experience with patients who have chronic or complex medical conditions preferred • Strong knowledge of healthcare resources, care coordination, and transitions of care preferred • Experience working in a remote or telephonic clinical environment preferred
