📋 Description

• Work with an assigned member population in Illinois over the phone • Perform health assessments, care planning, and education on chronic conditions and gaps in healthcare plans • Employ strategies, approaches, and techniques to manage members' physical, environmental, and psychosocial health issues • Identify and resolve barriers to effective care • Continuously monitor patient care using assessments, data, conversations with members, and active care planning • Ensure patients progress toward desired outcomes • Navigate multiple systems and use critical thinking skills • Provide personalized assessments, ongoing care planning, and expert guidance over the phone • Help members overcome barriers to care, manage chronic conditions, and connect with vital resources

🎯 Requirements

• Bachelor's degree in nursing (BSN) or health related field • Licensed Registered Nurse (RN) in the state of Illinois with no disciplinary action or ability to obtain IL RN licensure • Minimum of 3 years of clinical acute care experience as a licensed RN • Experience with health promotion, coaching and wellness • Bilingual in English/Spanish, English/Polish, or English/Creole (preferred) • Experience with case management, discharge planning and patient education for adult acute care (preferred) • Managed care experience (preferred) • Certified Case Manager (CCM) (preferred) • Prior experience with Medicare & Medicaid recipients (preferred) • Knowledge of community health and social service agencies and additional community resources (preferred) • Minimum download speed of 25 Mbps and upload speed of 10 Mbps • Wireless, wired cable or DSL connection is suggested • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
