• Deliver coordinated, patient-centered virtual Care Management by telephone and/or messaging to improve members’ health outcomes. • Generate impactful care plans with members and the multidisciplinary care team, helping members achieve their goals. • Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system. • Partner with members’ local providers to ensure coordinated care. • Provide compassionate, longitudinal follow-up care and build supportive relationships. • Guide members through routine and high-risk pregnancy experiences from preconception through postpartum. • Connect directly with members, listen to concerns, answer questions, and serve as a trusted advocate. • Support fertility planning, prenatal care, delivery preparation, and postpartum recovery. • Help members navigate health benefits and connect them with appropriate resources. • Work closely with providers, care coordinators, and support staff to deliver integrated remote care.
• Bachelor of Science in Nursing (BSN) • Must reside in a compact NLC state. • Active Compact RN license (in state of residence) in good standing with the nursing board of their state • Additional active Licensure a plus (IL, CA, MN, NY) • CCM Certification preferred • 5+ years of nursing experience • 2+ years experience working in care, case and disease management, preferably in a health plan, health navigator or third party administrator (TPA) environment • 2+ years experience working in labor and delivery • Be comfortable discussing a wide variety of medical conditions • Experience working remotely preferred; Comfortable with technology. • Be highly empathetic. • Must be able to work efficiently and meet volume goals without sacrificing quality. • Good judgment for balancing priorities. • Be flexible and comfortable with working in a rapidly-changing environment. • Strictly follow security and HIPAA regulations. • Be pleasant, responsive, and willing to work with and learn from the team. • Strong verbal and written communication skills. • Ability to gather a clinical history, answer questions at a patient level, and succinctly summarize findings. • Strong competence and ability to use multiple computer/medical record systems. • Collaborate well across multidisciplinary teams. • Ability to understand cultural and socioeconomic issues affecting members and coordinate available resources. • Excellent grammar, attention to detail, and ability to write medical information in easy-to-understand, patient-centric language. • Prompt and regular attendance at assigned work location. • Capability to remain seated in a stationary position for prolonged periods. • Eye-hand coordination and manual dexterity to operate keyboard, computer and other office-related equipment. • Occasional exertion of about 20 lbs of force may be required. • Capability to work with leadership, employees, and members in an appropriate manner.
