• Accurately reviews and codes patient records for hospitalist rounds and office visits with repetitive or non-invasive procedures • Reviews and analyzes medical records to assign ICD diagnosis codes, CPT procedure codes, modifiers, hierarchical condition categories, complications, and comorbidities • Evaluates diagnosis and procedure codes and modifiers against Outpatient Code Editor and National Correct Coding Initiative edits • Reviews clinical documentation to validate the patient's clinical picture, treatment, and diagnoses • Identifies missing, nonspecific, or inconsistent documentation and obtains clarification • Identifies and enters data elements for abstracting • Meets coding accuracy and productivity standards • Applies coding guidelines independently using discretion and analytic ability • Supports reimbursement, public reporting, quality of care, financial modeling, strategic planning, and marketing through accurate coding • Remains current with coding and industry changes through educational opportunities • Notifies coding leadership of trends and topics for physician and department education • Assists with process improvements to optimize resources, decrease costs, and improve coding services • Abides by AHIMA/AAPC Standards of Ethical Coding and official coding guidelines
• High School Diploma/GED - Required • Associate's Degree in Health Information Management - Preferred • Bachelor's Degree in Health Information Management - Preferred • 2 years Coding - Required • Surgery Coding experience - Preferred • CCS (Certified Coding Specialist) - American Health Information Management Association (AHIMA) - Required • RHIT (Registered Health Information Technician) - American Health Information Management Association (AHIMA) within 180days - Required -or- • RHIA (Registered Health Information Administrator) - American Health Information Management Association (AHIMA) within 180 days - Required • Thorough knowledge of professional coding guidelines, medical terminology, anatomy/physiology, and payer specific coding guidelines • Travel is required: Never or Rarely
