📋 Description

• Review hospital and professional inpatient or outpatient medical records to identify principal diagnoses, secondary diagnoses, and procedures • Examine laboratory findings, radiology and scan reports, discharge summaries, histories and physicals, consultations, orders, progress notes, and ancillary treatment records • Translate diagnostic and procedural phrases into coded form using procedure codes • Use Encoder software to determine diagnosis and procedure codes • Sequence codes to legally maximize reimbursement • Assign the appropriate DRG • Apply hospital and professional coding guidelines, Coding Clinic directives, federal regulations, CCI coding initiatives, CPT Assistant, and other standard coding guidelines • Query physicians to clarify documentation and facilitate complete and accurate coding • Review coding guidelines annually and recommend changes to improve coding and data management • Communicate new diagnoses, procedures, and technologies to the Coding Quality and Professional Manager • Update and correct historical file data by completing and submitting claim action reports per the PHC4 quarterly report • Satisfy job-specific obligations and comply with organization policies and procedures

🎯 Requirements

• High School Diploma or Equivalent (GED) (Required) • Minimum of 1 year related work experience (Required) • Relevant coding certification within 12 months • For level II and above, one relevant AHIMA or AAPC certification is required upon hire • CCA-only candidates must obtain a CCS, RHIT, or RHIA within 12 months of hire • Knowledge of AHIMA or AAPC coding certifications and applicable coding standards • Proficiency with Encoder software • Communication • Computer Literacy • Medical Records Management • Medical Records Systems • Teamwork • Working Independently • Must comply with coding guidelines, federal regulations, and organization policies and procedures • Does not qualify for J-1 waiver
