📋 Description

• Manage and maintain a caseload of applicant clients or caregivers through HCBA waiver enrollment • Provide ongoing case management services during the enrollment process • Track, chart, and maintain progress notes and case records according to agency policies and time guidelines • Document patient interventions and responses accurately • Maintain timekeeping and scheduling records • Collaborate with the RN on the Intake Team • Report signs of abuse or neglect to the appropriate agencies • Verify clients’ active Medi-Cal eligibility monthly and support applications when needed • Complete Institutional Deeming letters • Schedule client visits and RN visits • Document case notes within 24 hours and maintain contact information in MedCompass and NetSmart • Complete acuity assessments and other required assessments • Develop participant goals based on assessed needs, circumstances, and preferences • Identify external services for documentation on the POT • Provide information, education, counseling, advocacy, and support accessing waiver services • Assist patients and families in using family and community agencies • Establish care coordination schedules based on participant needs and acuity • Protect patient and agency confidentiality in accordance with HIPAA • Attend case conferences, staff meetings, and agency in-services • Participate in continuing education and maintain professional credentials • Respond to supervisor communications within 24 hours • Perform other assigned duties

🎯 Requirements

• Master's degree in Social Work or bachelor's degree in a related field • Minimum of two years of experience providing direct mental health or intensive case management • Experience working with clients with mental illness or chronic health issues • Proficient knowledge of managed care organizations, Medicare, and Medi-Cal • Excellent interpersonal skills and sensitivity to culturally diverse populations • Well-organized and able to comply with documentation and reporting requirements • Ability to maintain professional boundaries and manage time effectively • Current CPR certification • Ability to maintain an active license in the State of California • Ability to maintain all credentials up to date • Ability to perform required computer, telephone, keyboard, walking, lifting, pushing, pulling, and driving activities • Ability to lift and push/pull up to 25 pounds • Driving may be required in company-covered geographical areas
