📋 Description

• Provide member-centric, evidence-based case management across multiple products • Support members with the highest needs in navigating the healthcare system • Ensure safe transitions between care settings • Perform individualized assessments and identify members' goals of care • Coordinate services across various care settings • Connect members with community resources aligned with member needs • Reduce the burden of illness for individuals and families • Contribute to lower overall healthcare costs • Communicate with physicians and healthcare providers • Negotiate and access resources • Work efficiently toward department benchmarks • Perform other duties as assigned

🎯 Requirements

• Associate's degree in Nursing or equivalent work experience in related field • 3 years of work experience beyond degree in clinical/acute care nursing • Current, unrestricted RN license in the state of residence • Experience working with vulnerable and complex populations in a clinical, home care or telephonic environment preferred • Direct case management experience strongly preferred • Experience working with multiple age groups and diverse ethnic and socioeconomic backgrounds • General working knowledge of the health care continuum • Sound clinical foundation and familiarity with chronic conditions, ADLs/IADLs, and social determinants of health • Experience managing multiple computer systems and tools • Must be legally authorized to work in the United States at the time of application • Medica does not offer work visa sponsorship for this role
