• Lead the planning, implementation, operationalization, scaling, and continuous improvement of enterprise-wide programs across ambulatory, virtual, and specialty care settings • Partner with clinical, operational, digital health, informatics, quality, finance, and physician leadership teams • Oversee a portfolio of strategic initiatives focused on patient outcomes, care experience, access, and value-based performance • Establish program governance, stakeholder engagement, KPIs, workflow design, technology implementation, vendor management, regulatory readiness, and executive performance reporting • Develop program charters, roadmaps, implementation plans, business cases, budgets, operational models, policies, procedures, specifications, requirements, schedules, and status reports • Coordinate multidisciplinary teams and lead change management activities to drive adoption of new workflows and care models • Establish milestones, risk mitigation plans, and executive reporting processes • Assess operations and develop implementation strategies for readiness, compliance, and performance success • Identify process improvement opportunities and recommend corrective actions • Lead product and service go-live events and analyze market relationships and needs • Coordinate system-wide initiatives with vendors, informatics, and frontline service providers • Standardize processes, reporting, and approaches across organizational divisions • Track and interpret KPIs and business-to-business operational data for regional, market, and system management • Support quality control standards and policy work, including reporting, control evaluation, remediation, and monitoring • Facilitate and lead teams, groups, meetings, and discussions • Maintain knowledge of healthcare trends, compliance regulations, standards, and directives • Provide feedback to subject matter experts on products and services and suggestions for future program development
• 4 year/ Bachelors Degree, Specialty/Major: Healthcare, business administration, finance, analytics, informatics or related field (required) • 5+ years of experience in healthcare program management, clinical operations, population health, quality improvement or project management experience (required) • Chronic disease management knowledge • Value based care models • Healthcare operations and delivery • EMR systems (preferably EPIC) and integration • Program and project management skills • Ability to develop KPIs, dashboards and performance improvement plans • Leading through influence and leading multi disciplinary teams • Executive level stakeholder engagement • Change management • Scaling operational initiatives across multiple sites of care • Lean/principles • Verbal, written and presentation communication skills • Strong analytical, organizational and problem solving skills
