📋 Description

• Coordinate patient transfer logistics using client-defined workflows to support patient acceptance and bed placement • Identify the appropriate level of care, accepting provider, consultation, and access for referrals from hospitals, clinics, extended care facilities, and physician offices • Work directly with clinicians to communicate accurate clinical presentations, facilitate timely access and bed assignment, and document accurate patient information • Use critical thinking and clinical decision-making to develop appropriate destination plans of care • Use EHR, telephony, and office systems to support operations, ensuring accurate and timely data entry and protection of PHI • Make recommendations based on clinical information, patient or physician preferences, and transferring and receiving facility capability and capacity • Act as liaison between physicians, hospital staff, outside agencies, patients, and client facilities • Monitor individual and team compliance and performance • Support process changes and regulatory and quality initiatives • Apply knowledge of EMTALA, Ethics and Compliance, quality initiatives, and HIPAA • Report safety, compliance, and operational risks through the defined chain of command and recommend resolutions or improvements • Work with providers and healthcare professionals to ensure timely acceptance, access to appropriate care, and bed assignment • Participate in process improvement, professional development, peer review, and shared leadership • Assist with quality data collection, trend and report barriers to patient access and throughput • Perform other tasks and analysis as requested or directed

🎯 Requirements

• Active Registered Nurse (RN) licensure — required • Active multistate/compact RN licensure, if applicable based on state of residence — required • Associate Degree of Nursing or diploma (ADN) — required • 1 year of acute care experience — required • Must maintain active RN licensure within the states where transfer center services are provided • Designated workspace with a locked door, per HIPAA regulations • Secondary work location meeting HIPAA regulations recommended for power outages • Minimum internet speed of 100 Mbps download and 20 Mbps upload at primary and secondary work locations • Proficiency in EHR data entry, telephony systems, and office systems • Microsoft Office tools, including Outlook, MS Teams, Excel, and Word • Ability to review and interpret patient transfer case information • Ability to escalate or inform regarding patient status changes • Ability to record tasks and conversations regarding patient transfers • Critical thinking and clinical decision-making skills • Excellent oral and written communication skills • Bachelor’s degree of nursing (BSN) — preferred • 3+ years of acute care experience — preferred • Healthcare contact center experience — preferred • Epic electronic health record experience — preferred • Genesys contact center experience — preferred • Ability to work assigned shifts, including days, evenings, nights, every other weekend, rotating holidays, and on-call coverage • Ability to work the assigned schedule for at least six months before requesting a schedule change
