📋 Description

• Demonstrate clinical expertise for specific complex and/or rising risk member populations • Identify members appropriate for population health programs using data informatics • Provide interventions and education to promote health prevention and promotion • Enroll identified Trillium members into appropriate population health programs • Support the build-out and management of future population health programs • Engage members by identifying program mission, goals, and objectives and assisting with a thorough clinical background • Collaborate with the multidisciplinary team to develop tools and interventions for optimal patient outcomes • Coordinate and facilitate timely implementation of assessments, care plans, and appropriate interventions with Care Management staff • Follow established policies and procedures to achieve optimal clinical outcomes and improve Trillium members’ health status

🎯 Requirements

• Minimum of one (1) year as a Registered Nurse (RN) working with the BH/MH/SU/IDD population • Registered Nurse (RN) • Fully licensed by the North Carolina State Board of Nursing required • Must have a valid driver’s license • Must reside within North Carolina • Must be able to travel within catchment as required • Preferred: Knowledge of Quality Management, Utilization Management, and discharge planning procedures • Preferred: Experience using data analytics for population health management • Preferred: Certified Managed Care Nurse Certification or certification in care management • All candidates must pass a drug test as a condition of employment • Resume must include employer name, dates of service, average hours worked per week, essential job duties, education details, and licensure/certification information if applicable
