📋 Description

• Provide telephonic nutrition education sessions to members/patients with chronic conditions, including diabetes, heart disease and kidney disease • Collaborate with members/patients to explore important nutrition-related issues and topics • Connect members/patients to nutrition and condition resources from health plans, systems or communities • Document goals and care plans from education sessions in the platform • Schedule follow-up calls with members/patients • Develop nutrition education handouts and tools as needed • Identify community resources for clients • Maintain and update professional knowledge through educational opportunities, professional publications, networks and organizations • Contribute to social media content and video for company newsletter, website, articles, blogs and other posts

🎯 Requirements

• Bachelor of Science in Nutritional Science or Dietetics • Certification as a Registered Dietitian by the Commission on Dietetic Registration • Licensed or eligible for licensure in the state of residence (where license is required) • Minimum of 2 years’ experience providing nutrition education or counseling • Verbally fluent in Spanish with proficiency in medical and nutrition terminology in Spanish • Excellent oral and written communication skills • Strong organizational and interpersonal skills • Familiarity with standard office equipment such as laptop computers and smartphones • Ability to work independently with a strong sense of responsibility and minimal daily supervision • Ability to make clinical decisions regarding the appropriateness of nutrition education for each member/patient • Active licensure in District of Columbia or Ohio (preferred) • Previous experience providing telephonic nutritional education or counseling (preferred) • Training in health coaching, motivational interviewing and intuitive/mindful eating framework (preferred) • Ability to perform repetitive wrist, hand and/or finger motions • Ability to perform sedentary work involving prolonged sitting • Visual perception including peripheral vision, depth perception, and focus adjustment
