📋 Description

• Provide direct clinical care to patients with neurological conditions, memory disorders, and/or dementia via telehealth • Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians • Prescribe therapies • Partner with neurologists and the broader care team to ensure continuity of care • Educate patients and families on diagnoses, treatment plans, and disease progression • Maintain accurate, timely clinical documentation in the EMR consistent with regulatory requirements

🎯 Requirements

• Active, unrestricted Nurse Practitioner license in Massachusetts • Certification in geriatrics, adult-gerontology, or family practice preferred • 2+ years of clinical experience in neurology, memory care, and/or dementia care settings • Strong clinical judgement and clinical documentation skills • Comfortable managing a full patient panel independently with physician collaboration • Excellent communication skills with patients facing cognitive decline and their caregivers
