📋 Description

• Provide direct support services to NHS patients, including public school students • Use personal recovery experience to guide and support people in treatment and recovery • Work cooperatively with NHS providers and personnel to carry out organizational goals, policies, and governing requirements • Conduct patient intake interviews and assess social determinants of health and treatment/recovery barriers • Develop patient-centered plans based on strengths and goals • Follow up with patients by phone, home visits, school visits, clinic visits, or other settings • Assist patients with applications and registration forms • Identify, educate patients about, and refer them to appropriate community resources • Collaborate with clinical personnel and community partners • Communicate patient and family status and progress to the care team • Record patient care management information in the EMR or designated software within 24 hours of patient contact, when applicable • Manage an assigned patient caseload • Attend staff meetings, trainings, and other requested meetings • Perform other duties as assigned

🎯 Requirements

• High school diploma or equivalent required; Associate’s or Bachelor’s Degree in a human service program preferred • Certification through the Missouri Credentialing Board for Peer Support required, obtained within 2 months of employment if not already held • Community Health Worker certification obtained within one year of a supervisor’s request, if requested • 1 to 2 years of experience in a community-based setting preferred • Knowledge of some medical terminology preferred • Valid driver’s license and proof of insurance for use of a personal vehicle for business • Basic computer skills • Ability to work with all age groups functioning at different levels • Adequate, quiet, confidential remote workspace without distractions • Ability to adapt as agency needs change • Ability to share a personal recovery story involving substance use and/or mental health conditions • Ability to meet physical requirements including talking, seeing, hearing, reacting, standing, kneeling, walking, and sitting for long periods • Ability to read and interpret oral and written instructions • Patience, kindness, compassion, and good written and oral communication skills • Ability to maintain patient confidentiality • Ability to initiate and maintain positive working relationships • Professional appearance and demeanor • Compliance with the Missouri Credentialing Board’s Certified Peer Specialist Code of Ethics and any additional certification codes of ethics • Familiarity with SAMHSA’s Core Competencies for Peer Workers
