📋 Description

• Provide care coordination for members of the Children’s Behavioral Health Home • Contribute to program outcomes and improve health outcomes, cost effectiveness, and consumer engagement • Conduct work at members’ homes, providers’ offices, TOA locations, and other community settings • Collect data and participate in program initiatives • Attend team meetings • Coordinate service teams for each member • Use information to improve health outcomes • Complete documentation in a timely fashion • Develop and maintain a plan of care for each individual • Create and maintain therapeutic relationships with CBHH members to enhance wellness and increase community inclusion • Identify and obtain community resources from family members, medical providers, social workers, educators, and others • Establish client-centered, integrated care plans bridging medical and mental health care • Build effective service teams of professionals and natural supports • Meet program productivity and caseload expectations • Provide timely and accurate documentation reflecting clinical work and meeting licensing and contracting expectations • Work under the Director through staff meetings and individual supervision

🎯 Requirements

• Bachelor's in Social Work preferred or a Bachelor's Degree in a related field such as psychology, social services, counseling, rehabilitation, or nursing • At least one year of relevant experience working in human services with families and children required, preferably within the mental health system • Ability to work independently and collaboratively required • Ability to work flexible hours including evenings required • Must be able to meet physical requirements associated with family visitation, e.g. climb stairs • Computer proficiency with Microsoft Office and email required • For contracts that have Rider D: Must be able to successfully pass a criminal background, child protective service check, and sex offender check • Must not be on the state or federal suspension and disbarment list • Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, typing, and frequently lift and carry small children or material weighing up to 25 pounds • Driving may be necessary to attend meetings and trainings and occasionally meet with clients offsite and within the program service area • A good driving record is required • Must submit a cover letter and resume
