📋 Description

• Provide supervision for Nurse Practitioners delivering multidisciplinary care • Enable virtual healthcare for underserved patients using internal care delivery technology and clinical judgment • Provide virtual care to Medicare patients with chronic, complex, and overlapping medical, behavioral health, and social needs • Collaborate with physicians, nurse practitioners, care managers, and other care team members • Support care coordination and address patients' complex health needs • Help develop and refine holistic clinical programs • Provide input on clinical programs under development • Collaborate with and contribute to a diverse, inclusive, interdisciplinary team • Report to physician leadership • Work remotely for 5–10 hours per week initially

🎯 Requirements

• 2+ years of clinical experience serving patients with complex social and healthcare needs • Experience and desire to work closely with a multidisciplinary team • Technology-savvy and experienced in digital-first healthcare, such as telemedicine • Comfortable working across different technology platforms • Physically located in California • Quiet, HIPAA-compliant, internet-connected workspace • Board certification or eligibility as a Family Physician • Eligible to enroll in Medicare • Able to work with patients across the lifespan, from pediatrics to geriatrics • Experience working with Medicaid/FQHC clinics serving underfunded or underserved populations • Experience working with Medicare beneficiaries managing chronic disease • Medical license in California, plus additional state licensure or willingness to obtain additional licenses • Active California DEA license or willingness to obtain one • Startup experience is a plus
