📋 Description

• Evaluate and manage the changing health and service needs of patients • Provide optimum individual care for the patient/family unit using the nursing process and interdisciplinary team members • Collaborate with the patient's Primary Care Physician and other health team members • Create and implement written care plans within the time frame identified in documentation standards • Work as an integral member of the CHAT or ACO team

🎯 Requirements

• Washington Registered Nurse License upon hire • Bachelor's Degree in Nursing (preferred) • Three (3) years current experience in a health care environment with home health case management experience (preferred)
