📋 Description

• Manage specific patients throughout their admission • Ensure sufficient clinical and medical center resources are available • Communicate daily with medical staff, medical center personnel, and outside resources • Ensure appropriate utilization of resources according to clinical and financial standards • Monitor medical records for variances from clinical and financial standards • Intervene to achieve optimum outcomes for patients and the medical center • Apply risk management and infection control knowledge • Assist the healthcare team in achieving desired outcomes

🎯 Requirements

• Environmental Orientation due within 30 days of hire • Case Management Competency due within 90 days of hire • General Nurse Competency due within 30 days of hire • California Registered Nurse License upon hire • 2 years acute nursing experience, with evidence of progressive clinical experience preferred, or case management experience • Bachelor’s Degree in Nursing upon hire, with BSN required within 3 years of date of hire (preferred qualification) • CCM upon hire (preferred)
