📋 Description

• Provide a holistic, client-centered approach to caring for clients • Conduct assessments and continuous reassessments • Manage comprehensive care involving medical and cognitive challenges of aging • Develop and implement comprehensive care plans • Track, assess, and document client condition, preference changes, problems/opportunities, medical orders, family requests, and other care-plan information • Monitor, evaluate, and report on client status, actions/interventions, and outcomes • Work with formal and informal social support systems and families to meet client needs • Provide guidance, resources, and information to help families and clients make informed medical decisions • Manage electronic health records for clients, including billing • Coordinate resources for client care • Interact with physicians, attorneys, fiduciaries, hospice personnel, and community personnel on clients’ behalf • Create and maintain referral resources • Identify, coordinate, manage, and advocate for client services • Perform additional duties assigned by the supervisor • Provide assessment and monitoring, planning and problem-solving, education and advocacy, and family caregiver coaching

🎯 Requirements

• 5 years’ experience in community, hospital, hospice or home health case management • Bachelor’s or master’s degree in a healthcare-related field preferred • Certified Care Manager (CMC) or Certified Case Manager (CCM) • Demonstrated knowledge of aging, dementia, and/or disabilities • Experience working with complicated family systems and dynamics • Computer skills: MS Office, Electronic Health Records, and Internet skills • Excellent customer service and problem-solving skills • Excellent verbal and writing skills • Detail-oriented with the ability to keep up with fast-paced processes • Ability to lift and carry up to 10–15 pounds • Ability to sit, stand, and walk for prolonged periods throughout the workday • Frequent local travel by personal automobile to client homes required • Ability to use standard office equipment such as a personal computer, laptop, telephone, and photocopy machine • California-based applicants must provide timely and accurate positive fingerprint identification as a condition of employment • Must be able to work in the US, as indicated by “Fully Remote - US”
