📋 Description

• Provide end-of-life care to patients in their homes • Deliver compassionate, patient-centered hospice care • Collaborate with an interdisciplinary team focused on comfort, dignity, and quality of life • Respond to after-hours and on-call patient needs • Perform urgent assessments and manage symptoms and pain • Provide compassionate end-of-life support in real time • Support and guide patients and families during evenings, nights, weekends, and crisis situations • Coordinate care needs using clinical judgment and critical thinking • Respond to changing patient conditions • Maintain accurate, timely, and compliant EMR documentation • Work independently while traveling to patient homes within the assigned territory

🎯 Requirements

• Active Registered Nurse, RN license in the state you work • Two years of RN experience • Clinical expertise to deliver quality patient care • Hospice experience preferred • Current driver’s license • Ability to spend ~20% of your day driving to/from patient locations • Successful completion of a drug screen prior to employment • Legally authorized to work in the United States
