📋 Description

• Coordinate access to care for clients and ensure timely, effective solutions • Serve as primary point of contact for professional referral sources and clients during intake • Assess client needs, determine eligibility, and coordinate services • Collaborate with clinical and administrative teams to address client challenges and expedite solutions • Maintain detailed records and comply with regulatory and organizational standards • Build and maintain relationships with community organizations, healthcare providers, local agencies, and other referral sources • Collaborate with business development to identify and pursue growth opportunities • Represent Rosecrance at community events, networking functions, and professional meetings • Engage daily with internal and external stakeholders to align access and growth strategies • Participate in cross-functional team meetings • Spend at least one day per week in the community meeting referral sources and attending relevant events • Act as liaison between Rosecrance and the community • Comply with the Rosecrance Corporate Compliance Program, Code of Conduct, mission, vision, values, and expectations • Deliver exceptional customer service and serve as a positive role model • Perform other related responsibilities assigned by leadership

🎯 Requirements

• Bachelors Degree in Psychology, Social Work, Counseling, or related behavioral health field • Illinois Certified Addiction Counselor (CADC, or licensed in Illinois as a LSW of LPC) • Must qualify as a MHP per Illinois Department of Mental Health Rule 132 • Proven experience in behavioral health, social service, or related field • Experience in coordinating care with hospital systems, Employee Assistance Programs, or other safety sensitive populations • Strong problem-solving abilities and organizational skills to manage complex cases and coordinate care • Excellent communication and interpersonal skills, both written and oral • Ability to present and coordinate information for clients, staff, and community sources • Strong computer skills, including proficiency in operating various software applications and navigating digital platforms • Familiarity with the Chicago and Rockford regions preferred • Valid driver’s license and ability to meet agency insurance requirements • Belief in the mission and vision of Rosecrance • Availability for some evening and weekend work • Must reside in Illinois • Must pass an occupational health screening • Must work in a zero-tolerance drug-free environment, including marijuana
