📋 Description

• Work face to face with injured workers and medical providers • Facilitate advocacy, progressive medical treatment, and timely return-to-work • Act as a liaison and provide communication between all involved parties • Manage complex medical conditions, treatment planning, and recovery from illness or injury • Serve as a patient advocate to support, guide, and coordinate care for injured workers, families, and caregivers • Assist injured workers in achieving recovery and autonomy through advocacy, communication, education, service-resource identification, and service facilitation • Identify appropriate providers and facilities throughout the continuum of services • Ensure resources are used in a timely and cost-effective manner to obtain optimum value for clients and reimbursement sources • Travel frequently while maintaining a home-based office

🎯 Requirements

• Must live in the Philadelphia, PA and surrounding areas or Cherry Hill, NJ area • Medical/clinical or rehabilitation knowledge and experience • RN licensure preferred • Bachelor's degree in a health or human services field required • One of the following certifications required: CCM, CDMS, or CRC • Valid driver's license required • High speed internet required • Successful completion of a motor vehicle record (MVR) check may be required • Ability to handle work-related stress • Ability to handle multiple priorities simultaneously • Ability to meet deadlines • Computer keyboarding ability • Ability to travel as required
