• Serve as a patient advocate for injured workers, families, and caregivers throughout the recovery process • Support, guide, and coordinate care for injured workers • Assist injured workers in achieving recovery and autonomy through advocacy, communication, education, service-resource identification, and service facilitation • Work face to face with injured workers and medical providers • Facilitate progressive medical treatment and timely return-to-work • Act as a liaison and provide communication between all involved parties • Identify appropriate providers and facilities across the continuum of services • Ensure available resources are used in a timely and cost-effective manner • Maintain a home-based office while traveling frequently throughout the assigned area
• Bachelor’s degree in a health or human services field required • One of the following certifications required: CCM, CDMS, or CRC • RN licensure preferred • Valid driver’s license required • High-speed internet required • Ability to work face to face with injured workers and medical providers • Medical/clinical or rehabilitation knowledge and experience • Clear and conceptual thinking ability • Excellent judgment, troubleshooting, problem-solving, analysis, and discretion • Ability to handle work-related stress • Ability to handle multiple priorities simultaneously • Ability to meet deadlines • Ability to use a computer keyboard • Ability to travel as required • Employment may be contingent upon successful completion of a job-related MVR check
