📋 Description

• Review the daily caseload and prepare technology for patient services • Navigate the daily schedule using modern technology • Partner with clinical in-home team members visiting patients’ homes • Establish welcoming connections with diverse patients • Conduct virtual adult and/or pediatric wellness visits • Perform health risk assessments, medication reviews and cognitive screenings • Provide patients with educational information about health and well-being • Evaluate vital signs with Sprinters and arrange blood draws • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner license in Louisiana • Prior experience performing wellness visits • High levels of professionalism, integrity, accountability, confidentiality, care and compassion • Willingness to solve problems in a revolutionary environment, including last-minute and out-of-the-box problem solving • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search
