Sprinter Health is a hybrid in-home and virtual primary care provider that expands access to proactive, preventive healthcare by combining community-based, W-2 “Sprinters” (trained phlebotomists with medical assistant and community health worker skills) who perform hands-on diagnostics and wellness checks in patients’ homes with a virtual clinical team of physicians, nurses, pharmacists, and care navigators. Their full-stack platform and operations close care gaps, deliver preventive screenings and diagnostics, coordinate medications and referrals, and scale longitudinal primary care and wellness visits for payer and provider partners and patients.

📋 Description

• Review the caseload and prepare technology at the start of the day • Navigate the daily schedule using modern technology • Partner with clinical in-home team members visiting patients’ homes • Connect with diverse patients and collect relevant information • Conduct virtual adult and/or pediatric wellness visits and health assessments • Perform health risk assessments, medication reviews and cognitive screenings • Provide patients with health and well-being education • Collaborate with Sprinters to evaluate vital signs, arrange blood draws and address patient needs • Provide care coordination and management, including preventive care interventions, medication management, specialist referrals, community resources and documentation

🎯 Requirements

• Board Certified as a Family Nurse Practitioner • Active Family Nurse Practitioner License in California • Credentialed Medicaid provider for the state of California (Medi-Cal) • Prior experience performing wellness visits • Professionalism, integrity, accountability, confidentiality, care and compassion • Willingness to perform last-minute problem solving and out-of-the-box thinking • Technologically savvy and comfortable using laptops or mobile devices for charting and HIPAA-secure messaging apps • Strong written and verbal communication skills • Ability to work independently or in a team environment • Pass national background check and valid clinical license search • Experience with Medicare and/or Medicaid Annual Wellness Visits • Virtual experience as a Family Nurse Practitioner • Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner
