📋 Description

• Conduct telephonic assessments for high-risk members • Develop individualized care plans and measurable health goals • Coordinate services between providers, behavioral health teams, community resources, and members • Educate members on chronic disease management, preventive care, medications, nutrition, and healthy lifestyle choices • Support transitions of care and remove barriers to treatment • Maintain accurate documentation and ensure regulatory compliance • Advocate for members and help them navigate the healthcare system • Improve long-term health outcomes for Medicaid and underserved populations

🎯 Requirements

• Active, unrestricted Texas RN license in good standing • Must reside in Texas • 3+ years of experience in community health, public health, chronic disease management, community nursing, or case management • 1+ year of experience in care coordination and disease management/patient education • 3+ years coordinating care with primary care providers • Outpatient case management experience within a health insurance, payer, or managed care environment • Preferred: Medicaid Managed Care experience (STAR, CHIP) • Preferred: Marketplace or Commercial Health Plan experience • Knowledge of NCQA, HEDIS, HHSC, and TDI standards • CCM or other healthcare certifications preferred • Experience with Epic or other EHR systems preferred • Availability for Monday-Friday, 8 AM-5 PM schedule
