• Serve as the primary liaison and coach for healthcare facilities within the candidate’s state • Identify opportunities to improve patient care and outcomes • Implement improvement activities and achieve healthcare goals • Collaborate with nursing homes to identify needs and drive measurable improvements in patient outcomes • Partner with local, state, and national organizations to connect providers and patients with relevant initiatives and learning opportunities • Analyze regional, state, and provider-level quality data • Facilitate learning collaboratives • Provide technical assistance to nursing homes implementing evidence-based practices • Engage healthcare partners and providers across the healthcare continuum • Ensure alignment with state and federal healthcare quality objectives
• Bachelor’s degree in public administration, public policy, public health, or a related field, required, or equivalent experience • Equivalent pathway may include 10 years of healthcare experience or an associate’s degree with 5+ years’ experience • Healthcare quality improvement experience required • In-depth knowledge of quality improvement principles and methodologies used in nursing home settings • Strong interpersonal and communication skills for building relationships with C-suite leaders, clinicians, administrators, and other health professionals • Familiarity with state and federal nursing-home regulations, CMS quality measures, value-based payment models, and accreditation standards • Ability to coach and facilitate improvement activities and provide technical assistance • Must establish an ICT account and obtain an active EUA ID if not currently held, including required training, within 60 days of the start date • Preferred: nursing home or long-term care experience • Preferred: master’s degree in public health, quality improvement science, health informatics, or a related field • Preferred: Licensed RN, LPN, MSW, or CPHQ • Experience working with multidisciplinary teams including physicians, nurses, pharmacists, and administrators • Ability to manage project timelines, meet deadlines, and produce detailed written reports • Experience translating complex data into actionable insights and narratives for clinical and non-clinical audiences • Must not require work-authorization sponsorship now or in the future
