📋 Description

• Coordinate patient referrals and admissions • Confirm patients meet admission and clinical requirements • Communicate with patients, providers, pharmacists, home health agencies, and internal teams • Provide ongoing case management, including help with infusion pumps, supplies, and care transitions • Educate patients and caregivers about therapy needs and available resources • Deliver responsive customer service and help improve team processes

🎯 Requirements

• Graduate of an accredited nursing program • Active Registered Nurse (RN) license in PA or Compact • Strong communication, organization, and time management skills • Proficiency with Microsoft Office • Ability to manage multiple priorities in a fast-paced environment • If working from home, cable broadband or fiber optic internet service with at least 10 Mbps download and 5 Mbps upload • Bachelor of Science in Nursing (BSN) preferred • Experience in home infusion, specialty pharmacy, case management, or home health preferred
