📋 Description

• Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk • Provide medical follow-up visits for chronic disease management and ongoing care • Perform cognitive screening and staging; escalate for further assessment where indicated • Prescribe and manage GLP-1 therapy for appropriate patients • Conduct advance care planning conversations • Document in the EHR to support accurate Medicare billing • Work alongside the CMO, a board-certified geriatrician, with full training and clinical support

🎯 Requirements

• Active, unrestricted NP license in California • Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging • Active DEA registration • Experience with Medicare documentation and E/M coding • Comfort with telehealth and EHR documentation • Reliable high-speed internet and a private, HIPAA-compliant workspace • Experience with comprehensive geriatric assessment • Familiarity with cognitive screening instruments (Mini-Cog, MoCA) • GLP-1 prescribing experience • Multi-state licensure
