📋 Description

• Serve individuals with serious mental illness (SMI) who are being released from a DAC facility • Ensure communication between all parties, including DAC and related divisions • Coordinate and work parallel with the Justice Reentry Coordinator for referrals to necessary services • Promote person-centered practices in treatment planning and evaluation • Educate others about various housing options/resources and their access

🎯 Requirements

• High school diploma or GED required • At least two years of progressive experience providing similar services to the populations served (MH/SU/IDD) • Must have a North Carolina Certified Peer Support Specialist (NCPSS) Certification • Experience with DAC populations is preferred • Proficiency with Microsoft Office
