📋 Description

• Conduct 30-minute personalized 1-on-1 coaching sessions using motivational interviewing and evidence-based behavior change strategies • Facilitate group coaching sessions and workshops • Provide ongoing accountability, evaluate progress, and adapt strategies to drive long-term member success • Administer screening tools and health measurements within scope of practice • Maintain clinical documentation and provide customized care plans • Serve as a behavior-change subject matter expert to the multidisciplinary care team • Use health conditions and trend data to guide members to appropriate care and eliminate barriers • Connect members with digital health tools, health plan benefits, employer wellness initiatives, and community resources • Resolve member concerns and conduct proactive outreach calls • Manage scheduling, appointment coordination, follow-ups, and an active member panel • Participate in wellness campaigns, incentive programs, and client events • Adhere to the NBHWC Code of Ethics, HIPAA, PHI/PII protection protocols, and IT security standards • Escalate clinical or technical concerns according to operating protocols • Participate in quality assurance reviews, call audits, and routine feedback • Perform additional duties as assigned

🎯 Requirements

• National Board-Certified Health and Wellness Coach (NBC-HWC) credential • Bachelor’s degree in Health Science, Kinesiology, Nutrition, Psychology, or a related field • 2+ years of health coaching experience, preferably within a clinical, healthcare, or digital health environment • Bilingual English/Spanish • Certified Diabetes Prevention Program (DPP) Lifestyle Coach, preferred • Experience delivering care via telehealth or working within a remote team structure preferred • Strong baseline knowledge distinguishing the clinical boundaries between health coaching, health education, and behavioral health/therapy • Proven customer service excellence with a track record of driving patient engagement and retention • Technical proficiency with Electronic Medical Record (EMR) systems, Salesforce, and Google Workspace preferred • Exceptional verbal and written communication skills with a professional, empathetic demeanor • Strong caseload management, multitasking, and acute attention to detail • Agile and resilient in a fast-paced environment; open to evolving workflows and continuous feedback • Willingness to adjust working hours based on member, company, and client availability needs • Dedicated, private, and professional workspace for virtual visits when working remotely • Prolonged periods of sitting or standing at a desk • Extensive daily time spent using a computer, headset, phone, and video conferencing tools • Manual dexterity for typing and computer entry; corrected vision, hearing, and speech within normal ranges
