📋 Description

• Drive a high volume of outbound patient calls daily, ramping from 5–20 patient connections per day in the first month to 22+ by month three • Build genuine rapport with assigned patients using Motivational Interviewing techniques • Assess patient risk levels and prioritize outreach across the patient panel • Help higher-risk patients improve and reduce avoidable ER visits and hospital stays • Collaborate with patients to develop SMART goals and patient-centered care plans • Support PCP visits, treatment adherence, care navigation, and routine assessments • Document patient conversations, status, progress, and next actions accurately • Apply clinical best practices and coordinate with the broader care team • Drive patient engagement, accountability, behavior change, and improved health outcomes

🎯 Requirements

• Bilingual or multilingual communication skills required • Demonstrated clinical experience working with patients with one or more chronic conditions • Experience developing patient-centered care plans and SMART goals that support patient health outcome goals • Behavioral Health/Mental Health Integration experience highly preferred • Motivational Interviewing techniques preferred, plus the ability to pick up structured clinical conversation frameworks quickly • Prior use of care management platforms or EHRs • Prior use of MS Office Suite, especially Teams and Excel • Experience with multiple EHRs and database management programs • Knowledge of HIPAA and patient privacy compliance • RN, LVN, LPN, MA, MSW, LCSW, Medical Case Manager, Care Manager, Health Coach, Community Health Worker, Wellness Coach, Nutritionist, or Health Outreach Specialist experience/background
