📋 Description

• Conduct comprehensive clinical intake assessments and review medical histories for new and prospective patients • Triage inbound referrals, evaluate clinical eligibility, and determine urgency to route patients to appropriate care pathways • Partner with physicians, infusion nurses, and billing/intake specialists to ensure smooth care transitions and onboarding • Review clinical documentation for compliance with prior authorizations, insurance requirements, and regulatory standards • Maintain timely and accurate clinical documentation in the Electronic Health Record (EHR) system • Provide patient education regarding care expectations, initial appointments, and next steps in treatment plans

🎯 Requirements

• Graduate of an accredited nursing program • Associate or Bachelor’s Degree in Nursing (BSN preferred) • Active, unencumbered Registered Nurse (RN) license in the state of Michigan (or Compact License) • Minimum of 2 years of clinical nursing experience • Experience in intake, triage, utilization review, home health, or outpatient clinical specialty preferred • Strong clinical assessment capabilities • Excellent verbal/written communication • Proficiency with EHR software and office technology
