📋 Description

• Provide day-to-day leadership, guidance, and workflow coordination for the centralized Patient Services Coordinator team • Serve as supervisor, subject matter expert, and escalation resource for complex scheduling and patient coverage needs • Perform Patient Services Coordinator responsibilities while supporting the broader team • Monitor staffing coverage, productivity, scheduling effectiveness, and key operational performance measures • Review reports and dashboards to identify scheduling gaps, missed or incomplete visits, and coverage concerns and drive resolution • Partner with Branch Directors, Clinical Managers, and field clinicians to improve productivity, scheduling optimization, and patient coverage • Monitor visit utilization and scheduling patterns to reduce avoidable LUPAs and maximize clinical resources • Balance workloads and staffing across assigned agencies • Provide onboarding, training, coaching, and ongoing development for Patient Services Coordinators • Conduct quality reviews of scheduling activities and documentation for accuracy, consistency, and compliance • Improve workflows, standardize processes, and create efficient scheduling practices • Facilitate communication among centralized scheduling, branch leadership, clinicians, patients, physicians, and the interdisciplinary care team • Communicate operational trends, barriers, and opportunities to leadership and help develop solutions • Support QAPI, performance improvement initiatives, and operational projects related to scheduling and workforce optimization • Foster accountability, collaboration, urgency, and genuine caring across the team

🎯 Requirements

• Minimum of 3 years of healthcare experience, preferably within home health, hospice, clinical operations, scheduling, or care coordination • Previous experience in a leadership, team lead, training, coaching, or mentoring capacity preferred • Graduate of an accredited Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program preferred but not required • Previous home health experience and familiarity with clinical scheduling, visit utilization, and clinician productivity strongly preferred • Proficiency with electronic medical records, scheduling systems, and reporting tools • Strong leadership, coaching, relationship-building, organization, analytical, problem-solving, written communication, and verbal communication skills • Current LPN/LVN license in the applicable state for clinical candidates • Current CPR certification for clinical candidates • Ability to maintain applicable professional and regulatory standards • If travel is required, a valid driver's license, reliable transportation, and automobile insurance in accordance with agency requirements are required
