📋 Description

• Use Vori Health behavior change solutions consistently • Implement coaching techniques to improve patient outcomes • Provide video consultations and messaging to improve patients’ wellness goals • Design and support patient care plans using motivational interviewing, SMART goals, coaching best practices, and behavior change techniques • Manage an ample patient caseload • Document interactions and interventions using Vori Health technology tools • Collaborate with cross-functional teams and interdisciplinary clinicians • Engage regularly with physicians, physical therapists, advanced practice providers, dietitians, and others • Troubleshoot issues, prioritize patient needs, and avert gaps in care • Match patient needs to appropriate providers and stratify referral urgency • Support technology and service innovations for closed-loop communication and care coordination • Provide and receive performance feedback • Communicate and collaborate with external clinicians • Maintain professional and technical knowledge through workshops and professional publications • Meet company and quality standards • Perform other assigned projects and duties

🎯 Requirements

• Board Certified Health and Wellness Coach • Proficiency with computers, virtual care, database management, and data tracking • Knowledge of Motivational Interviewing, SMART goals, and behavioral stages of change • 2+ years of experience as a health coach and/or wellness educator conducting individual health consults or group workshops • Competence and passion for digital care delivery or telehealth • Comfort evaluating and treating patients remotely using digital tools • Outstanding written and spoken communication skills • Proven record of matching patient needs to treatment plans and implementing plans safely and effectively • Proficient with Microsoft Office Suite or related software • Reliable high-speed internet connection • Dedicated private home office space free from distractions and noise • Ability to work in a fast-paced startup environment with constant change • Excellent time management and ability to meet deadlines • Authorized or able to provide required documents to work in the United States • Willingness to work shifts that meet patient demand
