📋 Description

• Support members in meeting health goals and company Wellness program objectives through virtual health coaching sessions and diabetes education • Serve on both the Diabetes and Lifestyle coaching teams, with hours allocated according to time of year and client need • Help members create and meet health goals and objectives • Adapt coaching and education methods to members’ physical, psychological, cultural, and age-specific needs • Demonstrate established customer service skills • Understand each company’s wellness programs and keep members on track to meet program goals • Follow established program policies and procedures to support consistent coaching practices and program quality • Coach members on sleep, movement, nutrition, stress management, weight management, tobacco cessation, and improving biometric values • Provide health coaching and diabetes education for members with prediabetes, type 1, and type 2 diabetes • Review member-reported health data, including glucose readings, to identify patterns and trends • Help members understand their health data and encourage appropriate follow-up with personal healthcare providers • Provide education and coaching aligned with ADA Standards of Medical Care in Diabetes, AHA Hypertension Clinical Guidelines, and CDC Diabetes Prevention Program lifestyle recommendations • Work Monday through Friday, 7 hours per day with a 1-hour lunch break, between 8am and 8pm EST • Establish a consistent weekly schedule with the supervisor upon hiring

🎯 Requirements

• Bachelor’s degree in Nursing, Nutrition, Health Promotion, Public Health, Health Education, Exercise Science, or related field (required) • National Board Certified Health and Wellness Coach (required) • Registered Nurse or Registered Dietitian Nutritionist (preferred) • Certified Diabetes Care and Education Specialist (preferred) • CHES/MCHES (preferred) • Strong understanding of HIPAA compliance • 2+ years with direct experience in diabetes care and education • 2+ years of experience in direct health education or health coaching with individuals for disease prevention, health promotion, and behavior change • Experience working with digital health technologies, coaching platforms, and Microsoft 365 (Word, Excel, Outlook, Teams) • Knowledge of disease prevention and management, exercise principles, nutrition, and weight management with and without GLP-1 medications • Ability to work both independently and as part of a team with enthusiasm, initiative, and creativity • Flexible and able to balance the demands of Diabetes coaching and Lifestyle coaching • Bilingual in English and Spanish is a plus • Must work from a designated home office and not a public location • Employee must provide their own internet access
