📋 Description

• Conduct intake interviews to determine member needs and provide personalized support • Advocate for members by connecting them with social, medical, and community-based services • Monitor and document member progress and conduct consistent follow-up • Collect, analyze, and summarize treatment results and compile statistics • Prepare accurate and timely reports for Wider Circle leadership • Collaborate with community partners, healthcare providers, and social service organizations • Educate members about available resources and guide them through relevant processes

🎯 Requirements

• 3-5 years of experience working in the insurance industry • Experience in case management, social work, or healthcare advocacy • Excellent customer service skills • Strong written and verbal communication skills • Ability to explain complex policies in a straightforward, easy-to-understand manner • Ability to manage multiple priorities in a fast-paced environment • Conflict resolution skills and people management are required • Ability to build trusting relationships • Ability to collect, analyze, and report on data • Knowledge of community-based services and social programs • Commitment to the values of LOVE, LEARN, GROW and passion for supporting vulnerable populations
