• Provide support to patients and health care providers regarding Patient Support services, specific diseases, and therapies • Assist patients and caregivers with assessments related to Patient Support services • Liaise between patients, families, and health care providers regarding medication access • Address program, disease-state, and product-related inquiries using provided materials • Offer continuous support to promote patient adherence • Refer patients to healthcare providers for clinical questions beyond program materials • Document patient information and interactions in electronic patient records • Conduct ongoing file reviews and updates • Follow case-management protocols, including data collection, patient follow-up, therapy disruption/discontinuation monitoring, adverse-event reporting, and liaison with primary healthcare providers • Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternatives • Coordinate and track patient services according to KPIs to expedite therapy access • Participate in specialized therapy training sessions • Attend pharmaceutical meetings, patient meetings, and conferences; work with patient associations and advocacy groups • Coordinate with healthcare providers, pharmacies, and infusion/injection clinics for treatment logistics and issue resolution • Maintain clear communication with the manager, team, and client regarding patient cases • Assist in training new Nurse Case Managers • Provide feedback on protocols, databases, processes, and inter-departmental processes • Report adverse events and severe adverse events according to program procedures • Perform other assigned duties; may be required to work standby/on call
• Baccalauréat ou diplôme d’infirmier(ère) autorisé(e) (IA), diplôme d’études supérieures ou expérience spécifique de gestion d’un cabinet est préférable • A Diploma in health care or a related field, with a preference for Bachelor’s degree • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields • At least two (2) years of experience in public or private third-party reimbursement context, sales, care management, clinical support in the pharmaceutical industry, or related experience • Fluency in French and/or English may be required depending on program and geographic location • Proficiency in internet research and computer skills • Strong oral and written communication, problem-solving, teamwork, telephone, analytical and mathematical skills • Ability to communicate and understand medical information • Ability to handle sensitive issues involving conflicting viewpoints and strong negotiation skills • Effective interpersonal and leadership skills • Organizational skills and attention to detail • Ability to consistently meet deadlines • Proficiency in presentation techniques • Proficiency in Microsoft Word, Excel, PowerPoint, and other Office programs • Willing to travel at least 15% of the time each year
