📋 Description

• Provide care coordination and case management services for patients with acute and chronic conditions. • Serve as the primary point of contact for patients, families, and multidisciplinary clinical teams. • Support, educate, and advocate for patients, ensuring their individual care needs are met. • Coordinate care plans, monitor patient progress, and encourage adherence to treatment regimens. • Utilize EMR/case-management software to document and monitor patient care. • Collaborate with healthcare providers and insurance companies as needed. • Uphold the organization’s core values of teamwork, resilience, open-mindedness, ownership, and positivity.

🎯 Requirements

• Education: A Bachelor’s degree in Nursing, Social Work, Healthcare Management, or a related field is preferred but not required. Candidates with comparable experience or alternative degrees are encouraged to apply. • Experience: Experience in customer service, care coordination, or case management is preferred but not required. • Licensure: Nursing professionals with an active RN license are encouraged to apply, but licensure is not required for all candidates. • Certifications: Case Management Certification (CCM) is a plus. • Proficiency with EMR and/or case management software is beneficial. • Excellent communication, organization, and time-management skills. • The ability to work collaboratively within a multidisciplinary team. • A patient-first mindset rooted in empathy, integrity, and creative problem-solving.
