📋 Description

• Conduct comprehensive clinical assessments and develop individualized care plans • Coordinate inpatient reviews and discharge planning • Advocate for members and connect them with appropriate services and resources • Monitor care plans and adjust strategies to ensure optimal, cost-effective outcomes • Use motivational interviewing and coaching to support behavior change and engagement • Identify quality of care issues and contribute to program improvements • Deliver exceptional member service in a remote environment

🎯 Requirements

• Active, unrestricted RN license • 3+ years of acute care RN experience • Strong clinical judgment and decision-making skills • Excellent communication, organization, and multitasking abilities • Proficiency in Microsoft Office tools • Compact RN license preferred • Certified Case Manager (CCM) preferred • Case management or managed care experience preferred • Telephonic care or customer service experience preferred • Oncology care experience preferred
