📋 Description

• Receive and process new referrals from hospitals, physicians, and community sources • Verify patient eligibility, insurance coverage, and documentation for Medicare certification • Obtain and review physician orders before start of care • Coordinate timely start-of-care visits in accordance with Medicare Conditions of Participation • Act as liaison between patients, families, physicians, clinicians, and community resources • Establish, update, and monitor patient plans of care according to CMS guidelines and physician orders • Monitor visit frequencies, missed visits, and outcomes • Communicate changes in patient condition or physician orders to relevant team members • Enter patient data accurately into the Electronic Health Record system • Maintain Medicare documentation, including OASIS assessments, plans of care, and visit notes • Participate in Quality Assurance and Performance Improvement activities • Review clinical documentation before submission for billing • Assist with internal and external audits and regulatory surveys • Schedule RNs, PTs, OTs, STs, and HHAs based on patient needs, clinician skills, and visit requirements • Communicate schedule changes, missed visits, and urgent patient needs • Collaborate with clinical leadership to ensure staffing coverage • Provide service to patients, families, referral sources, and staff • Address and resolve patient or caregiver concerns • Promote a positive agency image in the community and among referral partners

🎯 Requirements

• Must be a US Registered Nurse with New York License • Minimum 2 years of experience in home health care, case management, or clinical coordination • Knowledge of Medicare Conditions of Participation • Knowledge of OASIS documentation • Knowledge of HIPAA compliance • Strong organizational and time-management skills • Ability to manage multiple priorities • Excellent verbal and written communication skills • Proficient in Home Health EHR software (e.g., Net Smart, Homecare Homebase, MatrixCare, or similar) • Detail-oriented and able to ensure accuracy in documentation • Empathetic, patient-centered, and committed to quality care • Reliable computer • Noise-canceling headset • Second monitor • Stable internet connection • Backup internet connection • Ability to work US Eastern Standard Time (EST), 9am to 6pm
