📋 Description

• Respond to all new referrals within 30 minutes of receipt • Match families with appropriate providers based on language, schedule, and location • Coordinate scheduling between families and providers until services begin • Maintain consistent follow-up and communication with all parties • Track referral status, outcomes, and reasons for lost referrals • Report recurring issues or service gaps to the team • Maintain accurate documentation for reporting and audit readiness

🎯 Requirements

• Fluent in English and Spanish (written and verbal) • Strong communication and follow-up skills • Highly organized with the ability to manage multiple tasks at once • Sense of urgency and ability to work in a fast-paced environment • Comfortable using systems to track data and manage workflows • Strong problem-solving skills and ability to make decisions independently
