• Support the District Director of Clinical Services (DDCS) in managing healthcare operations of communities in an assigned district • Ensure residents’ healthcare needs are met in compliance with state and federal regulations and Brookdale standards • Travel weekly throughout the district to assigned communities • Fill the HWD role when an HWD is on leave, extended vacation, or the position is vacant • Provide clinical support and guidance for communities with regulatory deficiencies • Audit community clinical compliance and partner with leaders to design and implement action plans • Perform root cause analysis and hands-on support for clinical outcomes and key compliance areas • Lead clinical initiative and rollout training for districts and individual communities • Coordinate and sign resident assessments in some states • Facilitate RN delegations for medications and care per state regulations • Participate in division, district, and community meetings • Train, coach, and guide clinical leaders and frontline staff • Facilitate Med Tech/QMAP/Medication Aide training and competency evaluations • Assist DDCS with clinical Quality Assurance site visits • Provide SWOT assessments and regular reports to DDCS • Support clinical investigations and potentially participate in disciplinary actions • Support DDCS with special projects • Provide district clinical support when DDCS is unavailable • Ensure timely and proper documentation under Brookdale policies and regulatory requirements • Adhere to company policies, procedures, and Clinical Systems • Apply the nursing process in accordance with state nursing regulations
• Associate’s degree in nursing, health education or related field or Diploma RN • Two or more years of direct nursing experience with older adults • Current nurse (RN) license, issued by appropriate state licensing agency • Valid driver’s license and access to a private vehicle for business use • Basic typing skills • Working knowledge of personal computing and word processing software, preferably in a Microsoft Windows environment • Ability to read and interpret documents • Ability to write routine reports and correspondence • Ability to speak effectively before groups of customers or associates of company • Ability to effectively communicate in English and understand and follow written and oral instructions • Ability to lift up to 50 pounds • Ability to travel frequently • Availability for occasional weekend, evening or night work if needed to ensure shift coverage • On-call availability on an as-needed basis
