📋 Description

• Develop, assess, and facilitate complex care management activities for members with primarily mental and behavioral health needs • Provide personalized care plans and education for members and families related to mental health and substance use disorder • Evaluate member needs by phone or in-home visits and recommend or facilitate care plans and service plans • Perform telephonic outreach to assess member needs and collaborate with resources • Develop ongoing care plans for members with high acuity • Identify providers, specialists, and community resources needed for mental health and substance use disorder care • Coordinate among members, families or caregivers, community resources, and care provider teams • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms • Facilitate care coordination and collaborate with providers or specialists to ensure timely access to care • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care quality and cost-effective delivery • Perform other assigned duties and comply with policies and standards

🎯 Requirements

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing • 2–4 years of related experience • Licensed Master's Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN based on state contract requirements • Behavioral health experience required • Ability to reside in the state of Missouri • Experience supporting behavioral health Medicaid populations ages birth to 64
