• Develop, assess, and facilitate complex care management activities for members with primarily physical needs • Create personalized care plans and provide education for members and families • Evaluate member needs, care access barriers, and social determinants of health • Develop ongoing care/service plans and collaborate with providers, specialists, and community resources • Identify problems and barriers to care and provide care management interventions • Coordinate members, caregivers, and care provider teams to ensure appropriate person-centered care • Follow up and monitor member status, condition changes, and progress toward care plan goals • Revise or update care plans with members, caregivers, and providers as needed • Provide resource support for employment, housing, participant direction, independent living, justice, and foster care services • Facilitate timely access to needed care and services • Conduct telephonic, digital, home, and other site outreach • Collect, document, and maintain member information and care management activities for regulatory compliance • Educate members and families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits • Provide leadership feedback on opportunities to improve care and quality delivery cost-effectively • Perform other assigned duties and comply with policies and standards
• Active New Jersey Registered Nurse (RN) license required • Degree from an Accredited School of Nursing or Bachelor's degree in Nursing • 2–4 years of related experience • RN state licensure and/or compact state licensure required • Ability to conduct in-home member visits and up to 50% local field travel • Availability generally Monday through Friday, 8:00 AM to 5:00 PM EST, with flexibility based on member and business needs • Knowledge of care management, healthcare coordination, member education, care plans, referrals, and healthcare benefits • Ability to document and maintain member information in compliance with state, federal, and third-party payer regulations
