📋 Description

• Develop, assess, and coordinate holistic care management activities for long-term care members • Develop or assist with personalized service care plans/service plans • Educate members and families/caregivers about available services and benefits • Evaluate member needs and available resources; recommend or facilitate appropriate plans • Identify providers, specialists, and community resources for long-term care • Coordinate between members, families/caregivers, and care provider teams • Provide resource support for employment, housing, participant direction, independent living, justice, and foster care needs • Monitor care plans, member status, and outcomes; recommend updates based on needs • Interact with long-term care healthcare providers and partners • Collect, document, and maintain member information and care management activities for regulatory compliance • Perform home and other site visits as needed • Facilitate education on procedures, provider instructions, service options, referrals, and healthcare benefits • Provide leadership feedback on quality of care and cost-effective service delivery • Perform other duties as assigned

🎯 Requirements

• Bachelor's degree and 2–4 years of related experience • Alternatively, Bachelor's degree in health, human, social work or education services with one or more years of qualifying experience • Alternatively, high school degree or equivalent and three years of qualifying experience with case management of the aged, including management of behavioral health conditions, persons with physical or developmental disabilities, or HIV/AIDS population • Equivalent experience acquired through applicable knowledge, duties, scope and skill reflective of the position level may qualify • Must reside within 30 miles of South Johnson, Miami, and/or Franklin County, Kansas • Computer savvy • Excellent communication and customer service skills • Knowledge of HCBS waivers and behavioral health • Strong time management and organizational skills • Ability to work effectively as part of a team • Knowledge of community-based referrals and resources • Strong problem-solving and assessment skills • Ability to work independently in a community-based environment • Ability to respond flexibly to client and program needs • Ability to work Monday through Friday, 8:00 AM to 5:00 PM, with evening and weekend work as required • Must comply with all policies and standards
