📋 Description

• Oversee care management for higher-acuity, long-term-care members • Develop and assess high-quality, cost-effective healthcare outcomes • Oversee care management teams' facilitation of member and provider needs throughout the care management cycle • Provide guidance and direct required long-term-care-management documentation for regulatory and contractual compliance • Review policies and procedures and present them to senior leadership • Monitor work assignments and caseloads based on state requirements, staff experience, and member needs • Manage and track team goals and objectives • Coordinate medical-service functions with other departmental functions • Participate in large or special projects • Identify process improvements and present recommendations to senior leadership • Review and monitor member data to identify trends and improve operating performance and quality care • Educate and provide resources to long-term-care-management leaders on key initiatives and member outreach • Coach and guide the team to improve member and provider experience and facilitate high-quality care • Manage and coordinate team-member training • Support strategy for onboarding, hiring, and training new team members • Perform other duties as assigned • Comply with all policies and standards

🎯 Requirements

• Candidates must reside in the state of Kansas • Bachelor's degree required • 6+ years of related experience • Prior management experience • Experience leading larger teams in a fast-moving environment highly preferred • Knowledge, duties, scope, and skills reflective of the level of the position, or equivalent experience acquired through applicable accomplishments • Ability to oversee long-term-care management for higher-acuity members • Ability to ensure compliance with federal and state regulations and contractual agreements • Ability to manage team assignments, caseloads, goals, and objectives • Ability to review member data and identify trends • Ability to coordinate onboarding, hiring, and training • Ability to comply with all policies and standards
