📋 Description

• Supervise the care coordination team serving long-term care members • Promote quality and efficacy of care management delivery • Supervise day-to-day escalations and care management issues related to members or providers • Monitor and review long-term care management documentation for compliance with federal and state regulations and contractual agreements • Assign caseloads and work assignments based on state requirements, staff experience, and member needs • Work with senior management on escalated and complex care cases • Provide guidance to junior team members addressing member concerns • Educate and provide resources on key initiatives and member outreach • Evaluate team performance and provide feedback on performance, goals, and career milestones • Coach and guide the team to improve member and provider experience and deliver high-quality care • Assist with onboarding, hiring, and training team members • Lead and champion change within scope of responsibility • Perform other duties as assigned

🎯 Requirements

• Bachelor's degree and 4+ years of related experience or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position • Candidates must be based in Illinois • LTSS and waiver experience preferred as ideal candidate qualifications • Prior lead or supervisory experience preferred as an ideal candidate qualification • Compliance with all policies and standards
