📋 Description

• Serve as a telephonic Registered Nurse and Care Manager for CenterWell Home Health • Ensure continuity of care for patients transitioning into and out of services • Provide expertise on post-acute levels of care and CenterWell home health, hospice, and palliative care services • Communicate with the CenterWell Home Health clinical team and facilitate patient follow-up • Support discharged patients and patients needing post-acute services • Navigate healthcare options, post-acute care services, Medicare coverage, billing issues, and healthcare resources • Provide patients with service-site options, specialty offerings, directions, and related education • Handle customer issues, complaints, and location lookups • Determine clinical level of care using discussions, medical records, and clinical data • Match patients to appropriate service locations or alternative services • Advocate throughout the referral process and maximize referral-to-admission conversion • Track referral and admission outcomes • Support department performance objectives and patient satisfaction goals • Assist with admissions as a patient ambassador • Provide education about Home Health, Hospice, and Palliative Care Services • Assist with clinical eligibility reviews for alternative services • Complete mandatory training and follow company policies and procedures • Participate in special projects and perform other assigned responsibilities

🎯 Requirements

• Associates Degree required • BSN preferred • Compact state RN license required; additional state licensures may be required based on business needs • At least 3 years post-acute nursing experience • Home health or hospice experience preferred • Knowledge of home health, hospice, and palliative care services • Ability to learn and master client locations and services • Analytical and problem-solving ability • Excellent verbal and interpersonal skills • Empathetic phone communication • Minimum home internet speed of 25 Mbps download and 10 Mbps upload • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information • Ability to work the specified 20-hour rotating schedule and holiday schedule • Ability to complete weekday evening and weekend daytime/evening training
