📋 Description

• Lead operational execution of new models of care for CKD Medicare Advantage patients • Lead a team of IKC RNs who case manage dialysis and CKD patients • Manage two supervisors and support non-clinical IKC care coordinators • Oversee quality and performance measures, including STARs metrics • Support RN hiring and oversee onboarding • Provide ongoing clinical guidance and training to RNs • Lead clinical and operational strategy with senior and cross-functional leadership • Manage Disease Management programs in the designated market or geography • Review and analyze data to drive patient care outcomes • Build and manage relationships with internal and external clients • Support Regional Operations Supervisors • Manage teammate HR activities, including payroll, PTO, hiring, onboarding, professional development, performance management, and disciplinary actions • Ensure regulatory and DaVita policy compliance • Monitor risk management and quality assurance compliance • Achieve quality and metric goals in Integrated Kidney Care programs • Coordinate with IKC management, Regional Medical Directors, cross-functional departments, RNs, and others • Evaluate processes for cost effectiveness and reduced operating expenses • Implement initiatives and analyze their impact on outcomes • Create and submit teammate performance reports and analyses

🎯 Requirements

• Minimum of 3-5 years’ management experience leading and managing teams • Demonstrated experience managing to data driven outcomes and process improvement • Primary Care and value based care experience • Strong communication on medical management, operational, and business-related issues • Intermediate computer skills • Proficiency in MS Word, Excel, PowerPoint, and Outlook • Home office with Internet connectivity at minimum of 1MB upload and 1MB download speed • Willingness and capability to lead remotely from a home office • Ability to travel as needed, including 1X a quarter and within the market • Residence in MDT/PDT/CDT time zones
